By signing this statement, in accordance with the Law of Ukraine “On Personal Data Protection,” I knowingly and voluntarily give my consent to the CHARITABLE FUND BGV CHARITABLE ORGANIZATION (hereinafter referred to as “the Fund”) to process (including collect, accumulate, store, and use) my personal data, as well as the personal data of my child/children/dependent(s) (hereinafter referred to as “Personal Data”), if my request concerns them, such as last name, first name, patronymic, passport details, taxpayer registration number, photograph or any other image recording, contact phone number, email address, information about the place of residence, and other data I voluntarily provide for the purpose of processing, ensuring the execution of civil and business relations, organizing charitable events, providing donations and other benefits (including in partnership with other individuals or organizations), informing and reporting publicly on such events, managing legal, tax, and accounting relations, and fulfilling other relations requiring data processing as per the Law of Ukraine, the Fund’s Charter and internal regulations.
I also consent to the transfer (sharing) of my Personal Data included in the Fund’s database solely for the purpose stated above and according to the procedures outlined by the Law of Ukraine “On Personal Data Protection” and the Fund’s internal policies on data processing and protection, including sharing data with individuals or organizations involved in fulfilling this purpose. I do not require notification of data transfer (sharing) of the Personal Data stored in the database to third parties if such transfer (sharing) is made for the abovementioned purpose.
By signing this statement, I confirm that I have been informed in writing about the inclusion of my Personal Data in the Fund’s database, the purpose of data processing (as stated in this document), the recipients of the data, and my rights under Article 8 of the Law of Ukraine “On Personal Data Protection.”
In the event that, based on my request, the Fund decides to initiate/organize/participate in/ or provide charitable assistance to me and/or my child/children/dependent(s), if the request concerns them, I give my unconditional consent for the Fund to record video and/or photos of me and/or my child/children/dependent(s). I also grant the Fund the right to edit the recorded materials and to use/ share/ publish/ or publicly distribute them indefinitely, including the use of my name and the name(s) of my child/children/dependent(s) in any media, social networks, television, exhibitions, etc., in any form (text, visual, audio, or audiovisual, etc.).
This consent is valid for an indefinite period.
